Presentation 1: Reducing Avoidable Readmissions Effectively (RARE) – A Quality Improvement Focus for Minnesota Health Care

Most of us have had a loved one – parent, spouse, child or friend – who had to return to the hospital soon after being discharged. We can identify with the anxiety, unrest and burden felt by all of those involved when this occurs.
Patients and their families would much prefer to remain in their homes and
sleep peacefully in their own beds. While many readmissions are necessary, a lot are avoidable. Nearly one in five Medicare patients discharged from hospitals in Minnesota is readmitted within 30 days. This is not a new problem but new tools are being developed through quality improvement to help organizations, systems and providers to improve this situation.

In this presentation, Kathy Cummings, will present:
o The current factors that contribute to readmissions,
o The quality improvement focus of the RARE campaign, and
o Tools that are being utilized to help organizations to decrease readmissions.

Presentation 2: HealthPartners Story: Applying Lean Principles in a Health Care Setting

HealthPartners is the largest consumer-governed, nonprofit health care organization in the nation. An integrated care and financing system based in Bloomington, Minnesota, HealthPartners has 12,000 employees dedicated to a mission to improve the health of members, patients and the community. HealthPartners has been using Lean process improvement techniques since 2004. Mary Russell, Director of Leadership Support and Organization Effectiveness, will share the story of HealthPartners Lean Journey including the successes and challenges of applying lean techniques in a health care environment.

About Our Speakers

Kathy Cummings, RN, MA

Kathy Cummings, RN, MA

Kathy Cummings, RN, MA, is a Clinical Systems Improvement Facilitator at the Institute for Clinical Systems Improvement (ICSI).

Ms. Cummings started her career as a staff nurse at the VA Hospital. Seeing the importance of staff development for improved care, she moved to Methodist Hospital to become an educator and eventually the Director of Education for Park Nicollet Health Services.

Throughout her career she has seen the importance of continually improving the work and the services an organization provides. In 2003, Ms. Cummings began the journey into Lean work and became Lean Certified. As a Lean expert she developed the first inpatient Lean office at Methodist Hospital in St. Louis Park, MN overseeing the integration of Lean improvement principles into patient care and supporting processes.

Ms. Cummings joined ICSI in early 2009. She is leading the RARE Campaign a project to reduce avoidable hospitalizations.

 

Mary Russell

Mary Russell is the Director of Organization Effectiveness and Leadership Support for HealthPartners. One of her major responsibilities is managing the Lean process across the all the HealthPartners entities which includes the Hospital, Medical Group and Health Plan divisions. She also has shared responsibility for HealthPartners Leadership Development and Support initiatives.
